General:

In order for Pet Express Transport Service to transport your pet(s) you must READ AND AGREE TO THE FOLLOWING TERMS AND CONDITIONS. A signed copy of this form must be received in our office prior to our drivers picking up your pet(s). We also have a Service Contract and two Boarding Forms that are required to be filled out and received by us before transport begins:

A: Medical Release and Disclaimer Form – which gives us permission to take your pet to a veterinarian if necessary while in transport with us. You agree to be responsible for any and all veterinary bills incurred in the event it should be necessary for your pet(s) to be seen by a veterinarian in route. Of course, all effort will be made to reach you so you can be the decision maker in your pets’ medical care. One form for each of your pets riding with us is required.

B: Feeding and Special Instruction Form – gives us feeding, care, and medication schedule if applicable to your pets’ needs. One form for each of your pets riding with us is required. ADDITIONALLY: ALL DOGS AND CATS MUST HAVE A COPY OF AN INTERSTATE HEALTH CERTIFICATE. THESE CERTIFICATES MUST BE
FROM A LICENSED VETERINARIAN NO MORE THAN 10 DAYS PRIOR TO SCHEDULED
TRANSPORT. ALSO, A COPY OF A CURRENT VACCINATION AND RABIES CERTIFICATE IS REQUIRED. These forms will be picked up with your pets on the day of transport and they will travel with your pets.

II. Payment - Half the cost of transport is required as a deposit, the balance of transport will be collected upon delivery of your pet family.

III: Pick–up and Delivery: Sending pets: IF NO PERSON IS PRESENT TO SEND pet(s) scheduled for transport and/or we are unable to contact the responsible party, the drivers will continue to transport without your pet(s). You will be charged for the full transport costs less additional insurance purchased. We recommend that you make alternate arrangements in case the person responsible for sending the pet(s) is unable to do so. Receiving Pets: IF NO ONE IS AVAILABLE TO RECEIVE the pet(s) and/or we are unable to contact the responsible party there will be a daily $100.00 fee per animal to defray the cost of care and boarding. The party that ordered the transport will be responsible for these extra charges. It is highly recommended that alternate arrangements be made in case the person
responsible for receiving the pet(s) is unable to do so. If you use a kennel or other boarding facility, it will be your responsibility to make sure they are aware that we may arrive outside of regular business hours so they can accommodate our schedule.

IV: Transport Duration:
Customers are provided with the estimated times for pick-up and delivery. It is understood that all estimates of length of transport is determined by origin, destination, weather and traffic conditions and other factors during any given transport.

Pet Express Transport Service is a 24-hour transport company and while every effort is made for our drivers to arrive at a convenient time, schedules may dictate arrival at any hour of the day or night. It is important to note that these are estimated times and situations beyond our control can create delays. If time delays (or advances) are significant, affected customers are called and updated with the revised estimated time of arrival.

V: Animals:
We cannot transport animals that have a tendency to be aggressive or vicious. If our drivers examine your pet(s) and determine that transportation of the animal(s) would be dangerous, we reserve the right to refuse transport. In addition, we will not transport any animal wearing a muzzle or other restrictive device.

VI: Animal Carriers:
It is important that the animal being transported be in an APPROPRIATE CARRIER. Pet Express Transport Service is bound by USDA regulations and wire cages are not allowed for either cats or dogs. All animals must have ample room inside of the carrier to be able to sit, stand, turn and lie down comfortably inside the carrier. We reserve the right to deny service to anyone not complying with there standards. Carriers being supplied by the customer must be in good repair, doors must close and latch properly.

VII: Rescheduling:
In the event, you find the need to reschedule your transport 10 days or less prior to your transport start date, a 5% rescheduling fee will apply. Please contact our office as soon as you know of a change of plan.

VIII: Cancellation:
Cancellation up to 1 week prior to your transport being DISPATCHED FROM ITS’
BASE OF ORIGIN will result in a 25% cancellation fee of the total transport cost.

Once transport has been DISPATCHED FROM ITS’ BASE OF ORIGIN cancellation will result in absolutely no refund.

BY SIGNING BELOW AND BY USING OUR SERVICES YOU AGREE TO BE BOUND BY ALL THE TERMS AND CONDITIONS SET FORTH IN THIS AGREEMENT.
